You obviously know how to write code. If you really want to write a custom import, I would consider writing a drush script that actually uses the drupal API to create nodes and populate them.

The only thing I didn't do what populate the corresponding field revision tables. But I really feel like I did this carefully, and am not sure what I am missing... | |

It's probably do'able; but you have to touch many tables beyond the mere content in order to keep drupal happy with node ids and whatnot. Start by searching for "drupal import." | |
