Steps

• Download and enable Chaos tools and Chaos Tools (CTools) AJAX Example
• Go to the URL your_site/ctools_ajax_sample
• You can refer ctools dialog form examples using sites/all/modules/contrib/ctools/ctools_ajax_sample
